How to access Health Insurance benefits

1. Since there are so many types of plans and benefit structures, the easiest way is to call our office with your insurance information. Alternatively, You can FAX a copy of your insurance card (both sides).
2. We will verify benefits directly with your insurance company. You will know your benefit package before any treatment begins.
3. Most plans require a deductible. We will also inform you of these features, so there are no surprises.
4. If you need a referral for massage therapy, you can obtain one from your MD, DC, or DO. If necessary, our office will FAX a referral form to your doctor in order to speed the process.

How to access Personal Injury Protection (PIP) benefits:

1. Call or FAX our office with your insurance information. You will need your insurance card, claim number, and insurance adjusters name and phone number.
2. You will need a referral from your treating doctor, whether it is a MD, DC, or DO. If necessary, our office will FAX a referral form to your doctor in order to speed the process.
